Multnomah County Health Department

Overview

The Multnomah County Health Department provides high quality health services for people who experience barriers to accessing health care.

Through culturally-appropriate and patient-centered care, we link physical and behavioral health and social services for low-income, uninsured and homeless people. We serve Multnomah County, Oregon’s most populous county with approximately 790,000 residents.

Services

Our health care services include primary care, dental and prenatal care; disease management for chronic illnesses; behavioral health; family planning; pharmacy; school-based health services targeted for children and adolescents; specialized care for HIV-positive individuals; and preventive services such as well-child health care, immunizations and nutrition services.

By integrating these health care services, we provide clients with coordinated care through our community health centers. This allows us to improve health for vulnerable populations.

Sites

We operate Oregon’s largest community health center. Services are offered throughout Multnomah County, from downtown Portland to North Portland and Gresham. Service sites include:

- 7 primary care clinics 

- 6 dental clinics 

-7 pharmacies 

- 12 school-based health centers

- 1 HIV health services center

Multilingual services are available in over 60 languages including Spanish, Russian, Ukrainian, Mandarin, Vietnamese, Lao and Arabic. 

Person-Centered Health Home

We have adopted the person-centered health home as our model of delivering health care.

Our clients enjoy improved health outcomes through a team-based approach that is proactive, person-centered and relationship-based.
