Founded 45 years ago as a Federally Qualified Health Center (FQHC), Hudson River Health Care (HRHCare) is one of the nation's largest community health providers. The HRHCare network of 43 health centers throughout the Hudson Valley, New York City, and Long Island provides affordable, accessible care to more than 245,000 patients annually. The network's exceptional primary care practitioners, specialists, and support staff have made HRHCare a destination for convenient, high-quality care for all.

HRHCare health centers provide primary and preventive treatment, including but not limited to: adult and pediatric care; obstetrical and gynecological care; dental services; behavioral health services; nutrition counseling; HIV care; health education; and a full complement of outreach, translation, transportation, benefits counseling, and health insurance enrollment assistance. Excelling in patient care, the HRHCare network has received the Gold Seal of Approval from the Joint Commission, the highest level of Patient-Centered Medical Home recognition from the National Committee for Quality Assurance and is a designated Planetree affiliate. HRHCare has a profound commitment to the communities it serves, and works to improve community health through partnerships and collaborations with community leaders, businesses, social service agencies, housing organizations, food banks, agricultural worker alliances, and other local stakeholders.

In December 2018, Brightpoint Health joined HRHCare. Prior to this event, Brightpoint Health was an FQHC network of 13 sites offering integrated medical, behavioral, and social support services to 45,000 patients annually in all five boroughs of New York City.
